Employee's Withholding Exemption Certificate available online to Alexander School District residents

Submission deadline is Dec. 18

The Alexander Local School District recently passed an operational levy that will implement a 1 percent school district tax to all its residents.  

Faculty, staff, and student employees that reside within the district need to complete a new State of Ohio Employee's Withholding Exemption Certificate to allow UHR to begin withholdings for this new tax in 2020. 
A new form must be completed in its entirety, further, affected employees must indicate that they reside in the Alexander Local School District and include the district’s number (0501) on the public school district of residence line which is the third line on form Ohio IT-4 and on the school district number line, also on the third line, respectively. To ensure that appropriate withholding for the entire tax year, a new form must be returned to UHR by December 18, 2019.

Completed forms should be returned to the UHR Employee Service center in person, by campus mail, or via fax.
